Township of Lawrence, NJ Job Details Part-time $113,614.37 - $136,825.91 a year 1 day ago Qualifications APRN Outpatient DEA License Advanced practice provider Geriatrics Full Job Description We are seeking an experienced Advanced Nurse Practitioner to join our geriatric outpatient practice one day per week (Fridays) . The ANP will independently manage a full panel of older adults, focusing on chronic disease management, acute visits, medication optimization, and preventive care. You'll work to manage up to 20‑patient clinic day. What You'll Do Provide geriatric primary care including chronic disease follow‑ups, acute complaints, and functional assessments Manage chronic conditions such as HTN, DM2, CHF, COPD, CKD, dementia, and frailty Perform medication reviews with a focus on polypharmacy and deprescribing Complete Medicare Annual Wellness Visits as needed Coordinate care with specialists, home health, PT/OT, and caregivers Complete visit documentation on EMR Participate in brief Friday huddles for case review and workflow alignment Qualifications Active
NJ APN/ANP
license National certification (AANP or ANCC) DEA registration (or eligibility) 1-2+ years NP experience (geriatric or primary care preferred) Strong clinical judgment and comfort with medically complex older adults
Pay:
$113,614.37 - $136,825.91 per year
