Compensation:
$53 - $56 hourly
Responsibilities:
Coordinate and manage comprehensive care plans tailored to each patient's unique needs, ensuring their comfort and dignity. Collaborate closely with interdisciplinary teams to deliver holistic care, addressing physical, emotional, and spiritual well-being. Conduct regular assessments and adjust care plans as needed, keeping the patient's best interest at heart. Educate patients and their families on care processes, empowering them with knowledge and confidence in their journey. Serve as a compassionate advocate for patients, ensuring their voices are heard and respected in all care decisions. Maintain accurate and timely documentation of patient care activities, supporting seamless communication across teams. Participate actively in team meetings, sharing insights and strategies to enhance patient care and team collaboration.
Qualifications:
Current Registered Nurse (RN) license in the state of New Jersey or Pennsylvania. Experience in hospice or palliative care, demonstrating a deep understanding of end-of-life care needs. Ability to develop and manage individualized care plans, ensuring patient comfort and dignity. Proven track record of effective collaboration with interdisciplinary teams to deliver holistic care. Strong communication skills to educate and empower patients and their families. Compassionate advocacy skills to ensure patients' voices are heard and respected in care decisions. Proficiency in maintaining accurate and timely documentation to support seamless team communication.
