Job Description
Per Diem MDS Coordinator Retirement Community | Medford, NJ $28.32 - 35.40 per hour (based on experience) The Minimum Data Set Coordinator works under the supervision of the Clinical Quality Manager and supports the interdisciplinary coordination of the Minimum Data Set (MDS) / Resident Assessment Instrument (RAI) process through collaborating with Nursing and other team members. The MDS Coordinator operates using MDS 3.0. Must possess a current, unencumbered license to practice as an RN in New Jersey.

$28.32/hr - $35.40/hrCore Responsibilities:
Works directly with the IDT members to establish timely and effective MDS/PPS assessment scheduling with the Director of Rehabilitation to fulfill OBRA and/or Medicare A/PPS requirements. Collaborates with IDT members in planning resident care, discharge and identification of any Significant Change in status of a resident. Completes nursing portion of the MDS 3.0 and conducts resident interviews as needed. Communicates with Skilled Nursing Resident Care Manager to support care plan process as outlined in the CAA triggers. Reviews all MDS's for completeness ensuring that IDT members have completed required portion of MDS 3.0.Collaborates with the Clinical Quality Manager to facilitate export/validation reports. Mentors and teaches nursing and IDT staff, as needed, to promote understanding of the RAI process. Maintains confidentiality of all resident care information including Protected Health Information, (PHI). Reports any known or suspected incidents of unauthorized disclosure to Corporate Compliance Liaison. Supports Clinical Quality Manager in state and federal survey process. As needed, participates in meetings and committees for Quality Assurance Performance Improvement (QAPI). Supports Clinical Quality Manager in completing tasks/duties as requested by Health Care management team.Education/Experience Requirements:
One year of long-term nursing experience required Preferred RNAC or MDS Coordinator experience Eligible or certified as a Resident Assessment Coordinator (certified) RAC-CT. Must possess a current, unencumbered license to practice as a RN in New Jersey.Who We Are:
