Job Description

Principal Accountabilities:
1. Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members. 2. Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements. 3. Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families. 4. Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; and answering questions. Deliver patient centered comprehensive diabetes education, supporting patients and families with self-management, treatment adherence, and continuity of care. 5. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; and following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. 6. Resolves patient problems and needs by utilizing multidisciplinary team strategies. Attend monthly TEAM and

meetings and quarterly
CLINICAL
meeting. 7. Maintains safe and clean working environment by complying with procedures, rules, and regulations; and calling for assistance from health care support personnel. 8. Protects patients and employees by adhering to infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations. 9. Documents patient care services by charting in patient and department records electronically (EPIC). 10. Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs in
EPIC. 11.
Maintains patient confidence and protects operations by keeping information confidential 12. Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; and evaluating new equipment and techniques. 13. Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; and using equipment and supplies as needed to accomplish job results. 14. Maintains professional and technical knowledge by attending educational workshops and participating in professional societies. 15. Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods. 16. Coordinate and complete prior authorizations and statement of medical necessity for medications, procedures, and medical equipment. 17. Process refill requests and triage patient calls to facilitate appropriate clinical follow-up. 18. Complete and document required direct patient diabetes education hours and continuing education units (CEUs) to meet eligibility requirements for the Certified Diabetes Care and Education Specialist (CDCES) examination, expected within two years of hire. 19. Develop and coordinate diabetes-focused educational initiatives, including CEU accredited programs for inpatient nursing staff and community healthcare providers, to support continuing education and evidence-based practice. 20.
