Job Description

Essential Duties and Responsibilities Include the following. Other duties may be assigned. In accordance with the Robert Wood Johnson AIDS Program patient care guidelines: provides medical case management for patient population includes HIV exposed infants, HIV infected children and adults, and pregnant women. Assesses the health status of patients assigned. Provides direct patient care for patients including vital signs, immunizations, STI treatment if necessary. Reviews medications and laboratory/imaging test results with physician. Enters lab orders and medication refills. Maintains communication with patient about disease status, medications, medication adherence, and disease education. Provides phone/text/email and/or written correspondence for patient appointments and missed appointments. Schedules all medical appointments and arranges transportation for patients if necessary. Prepares clinic visit prep form for outpatient visits. Documents all aspects of patient care. Provides referral and linkage information for patient appointment including: date, time, location of specialty visits. Prepares individual patient correspondence—letters to other physicians etc. Prepares medical information for patient paperwork i.e. SSI, Disability and ADDP. Obtains patient medical records as needed. Provides medical case management for HIV pregnant women in collaboration with prenatal providers. Provides coverage for other staff as needed. Manage assigned caseload of patients. Actively participate in quality improvement initiatives. Takes the initiative to understand the needs of all patients. Identifies barriers that inhibit patient care and works to remove them. Works towards achieving grant quality measures.
Patient Education:
Provides risk-reduction and HIV disease education for infected and affected patients. Provides medication education and medication adherence strategies for patient population. Provides disease education and disease management strategies for patients and families.
Data and Reports Requirements:
Provides data requirements for RWJAP including: daily patient encounter forms, new patient data form, monthly patient calendar, and others as needed. Provides statistical and narrative report data for quarterly/annual reports as requested. Prepares monthly perinatal updates for database. Maintains patient lab data and updates on restricted drive. General Demonstrates professional behavior through respect for patients, sensitivity to cultural differences, awareness of and attention to psychosocial factors that influence health, prompt completion of all patient-related paperwork, punctuality for meetings and patient care sessions. Acts courteously, compassionately, and responsively to all individuals. Collaborates with team members, colleagues such as other medical school personnel, referring agencies, and consultants in a manner that promotes teamwork. Understands and adheres to Rutgers' compliance standards as they appear in RBHS's Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy. Positively represents the school/unit to all customers. Maintains knowledge and attends continuing education training. Attends RWJAP weekly team meeting. Attends other program-related meetings as required and requested.
Participates in RWJAP special events:
World AIDS Day, HIV Testing events, and other program-related events such as patient holiday party. Performs other duties as assigned by Program Coordinator. Requirements Registered Nurse with at least three years of nursing experience.
Basic computer skills:
proficient with EMR, entering data on spreadsheets, Microsoft Word, and email. #IND2

$49.00 - $50.00 per hour
