Nurse Case Manager II (RN) Compensation Competitive pay rate: $44.14 per hour Nurse Case Manager II (RN) Highlights Monday-Friday, 8:00 AM - 5:00 PM Field-based and telephonic case management role Patient-facing position with onsite visits as needed Contract opportunity with potential for long-term career growth Individual start opportunity Candidates must reside in one of the following New Jersey counties: Monmouth, Morris, Somerset, Union, Passaic, Bergen, or Essex Nurse Case Manager II (RN) Responsibilities Conduct telephonic and face-to-face assessments in members' homes, hospitals, behavioral health facilities, and long-term care settings Develop cost-effective, member-centered care plans in collaboration with members, caregivers, and providers Monitor care plans and make adjustments based on clinical judgment and member needs Coordinate services and ensure timely delivery of care across the healthcare continuum Advocate for members during care transitions and discharge planning Address social determinants of health, including housing and food insecurity Collaborate with providers, caregivers, and interdisciplinary care teams Maintain accurate and timely documentation in electronic health records Utilize case management processes in compliance with regulatory and accreditation guidelines Mentor new team members as needed once proficient in the role Nurse Case Manager II (RN) Requirements Active, unrestricted RN license in the State of New Jersey required BSN required High School Diploma or GED required Minimum 3 years of clinical nursing experience required Healthcare and/or managed care experience required Strong assessment, critical thinking, and care coordination skills Proficiency navigating multiple computer systems and documentation platforms Excellent verbal and written communication skills Ability to work independently and manage a caseload effectively Must be an experienced and seasoned case manager Preferred Qualifications Case Management Certification (CCM) Managed Care Organization (MCO) experience Previous case management experience Experience coordinating care across multiple healthcare settings If you think this Nurse Case Manager II (RN) position is a good fit for you, please reach out!
