Full Job Description Function:
To serve as a direct support and liaison to program staff, individuals, and guardians by providing case management, consultation, and health education training relating to continued health and wellness.
Responsibilities:
Provides nursing case management to specific caseloads supported through agency programs; Liaises with program staff, individuals, guardians, and medical personnel who will be providing medical assessments, triage/acute care and treatment as necessary; Mange medication including bolus flush Review all past and current medical and medication records of individuals in care for licensing compliance; Monitors and tracks the health status of individuals during placement; Regularly monitors medication administration and medication counts in ID/DD programs to ensure quality and identify additional training needs; Conducts telephone follow-up and consultations with physicians, day programs, schools, and all program staff as needed, as directed; Facilitates medical referrals via primary care physician, as directed; Attends program monthly team meetings, annual ISPs and IDT meetings; Writes medical summaries at time of discharge when applicable; Provides ongoing and consistent training to program staff, individuals, and guardians through onboarding, compliance requirements, and professional development; Rotates on-call responsibilities; Performs other duties, as designed.
Qualifications/Requirements:
Minimum of a valid LPN license
Preferred:
BSN with valid New Jersey RN license A minimum of three (3) years nursing/case management experience, is preferred. Valid New Jersey driver's license and use of own vehicle for self-transportation. Excellent written and verbal communication and organizational skills. Knowledge of and sensitivity to cultural diversity issues. Ability to work effectively with people of diverse cultural backgrounds/races. Excellent interpersonal and relationship building skills. Good computer skills.
