REGISTERED
NURSE/CASE MANAGER
Position Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) Case Manager to join our team. The RN Case Manager is responsible for coordinating and managing the care of hospice patients, ensuring that patients and their families receive high-quality end-of-life care. The ideal candidate will be comfortable working independently in patients' homes and facilities while collaborating closely with the interdisciplinary hospice team.
Key Responsibilities:
Manage and coordinate the care of an assigned hospice patient caseload. Conduct initial patient assessments and develop individualized care plans. Provide skilled nursing care to hospice patients in homes, nursing facilities, and assisted living communities. Monitor patient condition and adjust care plans as needed. Educate patients and family members on disease progression, symptom management, and end-of-life care. Coordinate care with physicians, social workers, chaplains, home health aides, and other members of the interdisciplinary team. Ensure timely and accurate documentation in the electronic medical record system. Participate in interdisciplinary team meetings and care planning discussions. Provide emotional support and guidance to patients and families during the hospice journey. Conduct HHA and LPN supervisory visits
Qualifications:
Active Registered Nurse (RN) license in the State of New Jersey. Minimum 1-2 years of nursing experience required. Hospice or home health experience preferred but not required. Strong assessment and critical thinking skills. Ability to work independently and manage time effectively. Excellent communication and interpersonal skills. Pay:
$104,000.00 - $110,000.00 per year
