Flexible Schedule Position Overview:
A growing home health care provider in Atlantic County is seeking a Nurse Practitioner (NP) to join their team and provide high-quality care to home-bound patients. This role is ideal for a provider who is passionate about improving patient outcomes and delivering personalized care in a home setting. The Nurse Practitioner will work independently, managing a daily patient load while building strong relationships with patients and their families.
Key Responsibilities:
See approximately 5-6 patients per day in a home setting Perform comprehensive assessments including vitals, diagnosis, and treatment planning Prescribe and manage medications in accordance with clinical guidelines Maintain accurate and timely documentation and scheduling Educate patients and families on disease management, prevention, and overall health practices Collaborate with care teams to ensure continuity and quality of care
Qualifications:
Active New Jersey Nurse Practitioner license DEA certification Malpractice coverage Prior experience as an NP in clinic, nursing home, or ER settings Strong clinical judgment and ability to work independently Excellent communication and interpersonal skills
