Wellness Coordinator (Registered Nurse) Full Job Description Friends of Cyrus II located in Tabernacle, NJ is seeking to hire a Full-time dynamic IDD Wellness Coordinator , RN
MINIMUM QUALIFICATIONS
Minimum of 3 years' experience working with Intellectual Developmental Disabilities (IDD) Computer literate, including: Microsoft Office products, electronic health record, and electronic mail. Knowledge of IDD Terminology and NJAC 10:44A regulations. Willingness and ability to travel between multiple Group Home Locations. Experience with a NJ DDD provider agency or Home care agency
REQUIRED JOB RESPONSIBILITIES
Implement and monitor systems and agency wide trainings to reduce medication administration and documentation errors. Improve organization of medication and medical monitoring. Assists in a variety of initiatives and programmatic performance monitoring. Champions medication administration compliance; training, monitoring or medication comps after New Hire Orientation and annually. Monitors medical appointments and follow-up by creating processes and systems to ensure compliance. Reviews all Medication Administration Records (manual or electronic) for updates/completion at each location for each client on a continual basis or at least once every two weeks. Tracks and monitors the monthly Medication Check-in process and communicates with Innovative Pharmaceutical Packaging Corporation (IPPC - Agency Pharmacy) any concerns or issues identified. Leads in the rollout and monitoring of Therap (Electronic Health Care Record) functions related to medication and medical appointments. Conduct program audits and will document deficiencies as well as provide on the spot corrections for items deemed to be a safety or health risk. Will work with the programs to conduct quarterly audits to ensure compliance with the Plans of Correction (POC) and the NJAC 10:44A regulations (nursing section of audit tool). Provide staff in-servicing and training on topics directly correlating to the Wellness Director position (i.e., medication, diets, adaptive equipment.) Create reports deemed helpful for monitoring compliance related to topics and criteria that fall under the Wellness Coordinator position. Assist program staff in interpreting medical discharge paperwork as needed to determine appropriate follow-up required, the understanding of specific medical diagnosis and terms, supportive documentation needed as it relates to the standards and medication changes. Assists in the development and monitoring of medical documentation and the medical binders in the home for consistency. Participate in discharge meetings as requested for long term rehab stays and hospitalization to ensure a discharge plan is in place. Assess and monitor medication administration and develop process and monitoring systems to decrease the number of medication errors in the programs. Follow up on duties as assigned by the Chief Compliance Officer. Provides support related to improving health outcomes by focusing on the prevention of illness, injuries and deaths. The Wellness Coordinator will consult with treatment planning teams and guardians on issues related to health and safety. Attend meetings as directed by CCO, point of contact with physicians as required, ensure that all lab work is done timely, involvement in admission screening, point of contact with hospitals as it relates to medications, discharge plans, etc.
Requirements:
Unrestricted RN License. Masters Degree (Or equivalent with Certifications) CPR/First Aid/AED Certified. Minimum of one year supervisory or management experience in the healthcare field. Computer proficient with in-depth experience in Excel, Word & Outlook. Maintain a good driving record.
