Home Care Job Summary:
The Registered Nurse provides professional nursing care to patients in their homes through comprehensive assessment, planning, implementation, and evaluation of individualized care plans. This role ensures care is delivered in accordance with organizational policies, physician orders, and applicable regulations, while promoting safe, effective, and patient-centered outcomes.
Schedule:
Full-time position Day shift Occasional weekend and holiday rotation required
Key Responsibilities:
Deliver skilled nursing care through ongoing assessment, planning, intervention, and evaluation of patient needs. Ensure timely compliance with physician orders, including visit frequency and care plan adherence. Maintain accurate and timely documentation reflecting patient progress, interventions, and outcomes. Provide holistic care to patients and their families, ensuring safety and quality at all times. Perform treatments and procedures in accordance with established standards, policies, and best practices. Collaborate with interdisciplinary teams to coordinate care and support optimal patient outcomes. Apply sound clinical judgment and critical thinking consistent with professional standards and ethical guidelines. Manage authorizations and ensure compliance with payor requirements and visit frequency guidelines. Complete nursing assessments, medication reconciliation, and develop/update care plans as patient needs change. Educate patients and caregivers using appropriate teaching strategies aligned with care plan goals. Communicate patient status, concerns, and care updates to physicians, leadership, and care team members. Act as a liaison between patients, physicians, and the healthcare team to ensure coordinated and effective care delivery. Supervise home health aide staff, including completing required supervisory visits and contributing to performance evaluations. Maintain current knowledge of clinical standards and integrate best practices into patient care. Complete assignments independently, accurately, and within established timeframes. Perform additional duties and demonstrate flexibility as needed.
Physical Requirements:
Ability to lift, push, and pull up to 50 lbs. Ability to stand and be active for extended periods during the workday.
Qualifications:
Associate's Degree in Nursing or Diploma in Nursing required; BSN preferred. Minimum of six (6) months of RN experience or at least one (1) year of LPN experience, preferably in home care or hospice. Strong clinical, organizational, and communication skills. Basic computer proficiency.
Licenses & Certifications:
Active Registered Nurse (RN) license. Basic Life Support (BLS) certification.
Pay:
$95,000.00
