Job Description

Overview Join our dynamic healthcare team as a Registered Nurse specializing in Home Health services! In this vital role, you will deliver compassionate, expert nursing care directly to patients in their homes, promoting health, recovery, and independence. Your expertise in diverse clinical settings will empower you to assess patient needs accurately, develop personalized care plans, and coordinate with multidisciplinary teams to ensure optimal outcomes. Duties Conduct comprehensive patient assessments including vital signs, physiology evaluations, and medical history reviews to determine individual care needs. Develop and implement tailored nursing care plans aligned with best practices and patient goals. Provide direct patient care across various settings such as outpatient clinics, post-acute care facilities, or inpatient units, ensuring adherence to HIPAA regulations and medical terminology standards. Manage discharge planning and case management activities to facilitate smooth transitions between care levels. Perform diagnostic evaluations, monitor patient progress, and adjust treatments accordingly to optimize recovery. Educate patients and families on health management, medication administration, infection control practices, and disease prevention strategies. Document all patient interactions thoroughly in medical records and coordinate with healthcare providers for seamless continuity of care. Requirements Valid nursing license with current registration; experience as a Registered Nurse in diverse clinical environments preferred. Critical care experience including ICU, emergency room, or trauma center work highly desirable. Proven expertise in hospital medicine, behavioral health, geriatrics, or pediatrics; experience with Level I Trauma Centers or Level II Trauma Centers is advantageous. Strong knowledge of medical-surgical nursing, acute care procedures, and physiological assessment techniques. Familiarity with outpatient clinics, post-acute care settings, and managed care environments. Ability to perform patient assessments accurately while managing multiple priorities efficiently. Excellent communication skills for effective discharge planning and patient education. Knowledge of HIPAA regulations and medical terminology essential for maintaining confidentiality and documentation standards. Experience with diagnostic evaluation processes, utilization management, case management strategies, and inpatient/outpatient workflows preferred. $95,000.00 - $120,000.00 per year
