Reassess patients systematically to identify trends requiring intervention. Develop and implement individualized plans of care within established nursing standards and timeframes. Make nursing decisions based on evaluation of patient outcomes and modify the plan of care as needed. Provide and document patient/significant other education and support discharge planning. Implement and document timely interventions appropriate to patient needs. Perform point-of-care testing as required for patient care.
