DOE Primary Position Purpose:
The Position is a professional position requiring certification promulgated by the U.S. Department of Health and Human Services in the community health care field. This position will assist in the coordination of all compliance activities as agreed in the Federal Grant Scope of Work of the Community Health Representative (CHR) grant under the overall direction of the Department Director. Accordingly, the position requires a background in the health and medical field along with organizational, writing and communication skills. Primary Duties Deliver medications, supplies, and equipment to individuals. Provide safe transportation to and from hospital, clinic, pharmacy, and other sources of care for non-life-threatening problems. Provide emergency first aid care to individuals while arranging ambulance transportation and treatment. Provide health education and prevention activities to the community on a quarterly basis or as needed/required. Will assist the Director in working with Tribal Entities, The Indian Health Service and State Agencies to reduce the risk of disease and environmental hazards in the home and community. This includes but is not limited to scheduling flu shots or other vaccines, diabetes clinics, health fairs, water testing, smoke alarm checks, rabies vaccines clinics/ spay clinics and rodent control. Attend trainings to continuously update patient care techniques, knowledge and maintain certifications. Complete PCC forms on all patient/client contacts. Assist with and compile demographics for grant applications. Maintain daily records of all activities performed including expenditures, vouchers, vehicle maintenance, project and supplies and submitting receipts on a timely basis; as well as weekly vehicle inspections. Submit monthly reports to Dept. Director Knowledge Required Practical knowledge and experience managing community health events and programs. Experience and practical ability to manage public health grants and deliverables contained therein. Knowledge of agencies such as the IHS, and other related State Agencies and their function and overall relationship with the Pueblo of Pojoaque Community Health Program. Basic but practical medical knowledge of diabetes and diabetes related issues. Basic knowledge of patient transportation and vehicle maintenance. Physical Demands Position will require incumbent to be in good physical condition and overall health and must follow all safety policies and procedures. Must be able to pick up at least 50lbs. Work Environment/ Working Conditions Working conditions require work both in an office setting and outdoors including the safe operation of transport vehicles. The coordinator must be able to adapt to varying circumstances sometimes in a single day from providing class instruction to possibly needed transports, client care, in-home need, meetings, office work etc. This work may require extended hours.
Job Related Qualification Standards/Requirements:
Must possess a High School Diploma or GED equivalent Must possess a valid New Mexico Driver's license and be covered under the Pueblo of Pojoaque insurance coverage. Must possess verification in CPR and emergency medical treatment Must possess or obtain defensive driving certification. Must possess good interpersonal communication skills.
