POSITION PURPOSE
The Registered Nurse (RN) Navigator-Surgery position is a professional Registered Nurse with surgery clinical knowledge who offers individualized assistance across the care continuum to patients, families and caregivers to expedite and coordinate care and address health system barriers. The RN Navigator-Surgery position assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes. The RN Navigator-Surgery position has a particular focus on improving transitions in care and adherence to care and additionally, functions as a clinical advocate and educator for patients. The RN Navigator-Surgery position identifies and alleviates stressors during transitions and barriers to surgery with the aim of delivering a seamless model of care that benefits patients, family members, providers and the healthcare team.MINIMUM MANDATORY QUALIFICATIONS
EXPERIENCE:
Minimum three (3) years of clinical nursing experience.EDUCATION
Associate (ADN), Bachelor (BSN) or Master (MSN) Degree in nursing from an accredited college or university.MANDATORY KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS
Current and valid nursing license in the State of New Mexico, or compact state; maintained current throughout employment. Possess a strong professional, personable and motivational personality that builds strong relationships with patients. Strong knowledge and interest in current techniques, advancements and trends in the field of ambulatory care. Knowledge of computer applications, including the Microsoft Office suite of products and familiarity with electronic health record systems. Ability to establish and maintain effective working relationships with practice leadership, physicians, advanced practitioners, colleagues and other Holy Cross Medical Center (HCMC) and Taos Professional Services (TPS) team members. Excellent communication skills, including the ability to effectively interact with all members of the care team and with patients and family members. Basic Life Support (BLS) certification is required and maintained throughout employment.PREFERRED QUALIFICATIONS
Three (3) years experience in a specific service line (orthopedics, general surgery, urology, etc.) One (1) year surgical scheduling or surgical coordinator experience One (1) year ambulatory care experience One (1) year Case management or patient navigator experience Board Certification (RN-BC) maintained throughout employmentSimilar jobs in Taos, NM
