Full Job Description Job Summary:
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with physician orders, agency policies, state regulations, and Medicare Conditions of Participation. The LPN promotes patient independence while ensuring safe, compassionate, and high-quality care.
Essential Duties and Responsibilities:
Provide skilled nursing care as ordered by the physician and directed by the supervising RN. Monitor and document patient condition, vital signs, and response to treatment. Administer prescribed medications and treatments within the LPN scope of practice. Perform wound care, catheter care, ostomy care, and other skilled nursing procedures. Observe for changes in patient condition and promptly report findings to the RN and physician as appropriate. Reinforce patient and caregiver education regarding medications, disease management, safety, and home care instructions. Maintain accurate, complete, and timely clinical documentation in the agency's electronic medical record. Follow infection prevention and control policies. Participate in case conferences and communicate effectively with the interdisciplinary team. Maintain patient confidentiality in accordance with HIPAA regulations. Comply with all federal, state, Medicare, and agency policies and procedures.

$50.00 per hour
