Home Health Clinical Nurse (RN) Harmony Home Health is an established home healthcare company that has been servicing patients in Clark County for over 20 years is seeking a Home Health Clinical Nurse (RN)
Job Description Summary:
Oversees the home care for a number of patients. Normally makes the initial visit and follow-up visits as needed. Reports changes in the patient's medical or mental condition to the patient's primary care physician. Reviews patient's clinical diagnosis, OASIS, medications, procedures and clinical course. Coordinates and collaborates with the patient's physician, devises a care plan for the home disciplines to follow during routine visits. Implements, monitors, and evaluates patient's plan of care for appropriateness to individual patient needs. Responsible for ensuring that home patient care is coordinated and managed appropriately according to state and federal guidelines. Responsible for the clinical activities of the field disciplines. Manages disciplines to ensure that disciplines are adequately caring for the patient. Reviews discipline notes to ensure the patient's care plan is being followed and discussing concerns over patient care. Acts as mediator if conflicts between client or family and staff arises. Performs home supervisory visits of nursing services delivered by licensed nurses, home health aids and other nursing support staff. Assists and reports directly to the Director of Professional Services.Qualifications:
Must be a current licensed Registered Nurse in State of Nevada, with CPR certification and valid driver's license and means of reliable transportation. Current physical, TB test and COVID vaccines required. Must be able to pass a BCI background check. Prefer at least one year's experience working in home health, medical-surgical or critical care units. Previous supervisory experience may not be required. Must have positive attitude and strong self-motivated work ethic, professional clinical skills, excellent communication and interpersonal skills. Can demonstrate personal computer and electronic medical record knowledge and skills. Must be highly and consistently organized.Pay:
$60.00- $120.
