Monday Friday, 8:00 AM 5:00 PM (40 hours/week, no weekends) What You llBe Doing:
Providing primary care visits within an ISNP model Working across rehab centers, transitional care, and memory care facilities Managing a geriatric patient population Completing approximately 4 7 patient visits per day Practicing within a value-based care model utilizing HEDIS measures Charting inAthena Compensation & Benefits:
