You will collaborate closely with physicians, social workers, chaplains, aides, and other members of the hospice team to ensure patients receive comprehensive, patient-centered care wherever they call home. Key Responsibilities Perform comprehensive initial and ongoing patient assessments Develop, implement, and update individualized hospice care plans Manage pain and symptoms in accordance with physician orders and hospice best practices Provide skilled nursing visits in patient homes, assisted living communities, and skilled nursing facilities Educate patients, families, and caregivers regarding disease progression, medications, symptom management, and end-of-life care Coordinate care with physicians and interdisciplinary team members Participate in care conferences and interdisciplinary team meetings Advocate for patient comfort, dignity, and quality of life Maintain timely and accurate clinical documentation in compliance with hospice regulations Communicate changes in patient condition and coordinate appropriate interventions What We're Looking For Active, unrestricted Registered Nurse (RN) license Minimum one year of nursing experience required Hospice, home health, palliative care, oncology, geriatrics, or community-based nursing experience strongly preferred Strong clinical assessment and critical-thinking skills Excellent communication and relationship-building abilities Passion for serving patients with serious or life-limiting illnesses Ability to work independently while collaborating effectively with an interdisciplinary team Requirements Current RN license in the state of practice Current BLS certification (American Heart Association or American Red Cross) Valid driver's license, reliable transportation, and proof of insurance Ability to travel throughout the assigned service area Physical Requirements Ability to stand, walk, bend, reach, and lift up to 50 pounds Frequent travel between patient locations Ability to utilize EMR systems, tablets, and laptops for documentation Join a Team That Leads With Heart At Dahlia Hospice, we believe hospice is more than a service—it is a calling.
