JOB SUMMARY
The RN Care Coordinator for the Supportive Crisis Stabilization Center provides basic and essential medical care to clients admitted to the dually certified
OMH/OASAS
crisis facility. The SCSC is a 24/7/365 program for individuals in Mental Health/Substance Use Disorder (MH/SUD) crisis situations. The SCSC will provide immediate, therapeutic support by certified peers and licensed behavioral health professionals. The RN Care Coordinator is part of a multidisciplinary team of staff and will support triage, screening, assessment, interventions, discharge planning and follow-up services for clients. The RN Care Coordinator is responsible for providing quality care to clients utilizing a person-centered approach for all individuals.
DUTIES AND RESPONSIBILITIES
Assist in the daily operations of the certified SCSC supporting individuals with Mental Health/Substance Use Disorder (MH/SUD) challenges Adheres to all state requirements and professional nursing standards as a licensed provider of medical care and treatment Triage, assess, and observe clients to determine appropriate level of care for individuals presenting at the SCSC for admission, using evidence-based screening tools and a trauma informed approach Monitor medical status of clients, including vital signs, basic medical care, examination and observation of signs/symptoms of medical conditions Provide therapeutic interventions to clients in crisis including psychoeducation, crisis counseling, and de-escalation Consult with on call Psychiatric Provider for medical/clinical advisement on psychiatric and/or substance use related concerns of clients, as applicable or as directed Maintain accurate records and complete documentation of client services Coordinate the appropriate care and services for clients, including: Assist with the development of and ensure the implementation of Safety Plan and Individual Service Plan outlining care, treatment, and recommended services based on individual client needs at admission and upon discharge Provide information and referrals to treatment facilities or services for SUD Provide information and referrals to clinical/therapeutic services for mental health challenges Provide information and referrals for medical insurance and medical/primary care services Provide information and referrals to services related to social determinant of health needs (housing, financial, food security, transportation, childcare, utilities, safety, etc.) Collaborate with community partners to facilitate appropriate services upon discharge Communicate relevant client information with family members, caregivers, or designated individuals, as appropriate and if authorized Document information related to referrals, coordination of services, and follow-up contacts and supports provided to client and community providers Follow OMH and OASAS regulatory requirements and MHAST policies and procedures Help maintain overall cleanliness and safety of the facility; ensure a welcoming and supportive environment for clients Treat all clients with dignity and respect Demonstrate teamwork among co-workers and contribute to a multi-disciplinary team approach Attend agency staff meetings and trainings Cooperate with audits, inspections, investigations Flexibility to potentially cover shifts, in the event of call-ins or scheduling issues RN/Medical Care and Monitoring (30%)Coordination of Client Services (50%)Other (20%)
