Job Description
The Oncology Nurse Coordinator serves as a central point of contact for patients, families, providers, and multidisciplinary care teams throughout the continuum of cancer care. This role provides clinical coordination, patient navigation, education, advocacy, and support from initial diagnosis through treatment, survivorship, and/or end-of-life care when appropriate. The Oncology Nurse Coordinator assists patients in navigating complex diagnostic evaluations, treatment planning, oral and intravenous therapies, imaging, laboratory testing, supportive services, and follow-up care. The position promotes timely access to care, improves communication across disciplines, and enhances patient outcomes and experience. Essential Duties and Responsibilities Patient Navigation & Care Coordination Guide patients and families through the oncology care continuum from diagnosis through treatment and follow-up. Coordinate timely scheduling of diagnostic testing, imaging, biopsies, consultations, procedures, treatments, and follow-up appointments. Facilitate transitions between surgery, medical oncology, radiation oncology, primary care, specialty services, and supportive care. Track patient progress to ensure adherence to evidence-based treatment plans and timely interventions. Identify and address barriers to care including transportation, financial concerns, insurance authorization, psychosocial needs, and health literacy challenges. Assist with coordination of second opinions, referrals, and multidisciplinary tumor board recommendations. Monitor care timelines to reduce delays in diagnosis and treatment initiation Clinical Responsibilities Perform nursing assessments and ongoing symptom evaluation. Triage patient concerns, treatment-related toxicities, urgent symptoms, and side effects via phone, portal, or in-person communication. Provide clinical guidance within scope of practice and escalate concerns appropriately to providers. Review laboratory, imaging, and pathology results as directed by providers. Support management and coordination of systemic therapies including chemotherapy, immunotherapy, targeted therapy, hormone therapy, and oral oncolytics. Reinforce treatment plans, medication adherence, and symptom management strategies. Participate in survivorship planning and coordination of long-term follow-up care. Patient & Family Education Educate patients and caregivers regarding diagnosis, staging, treatment options, procedures, medications, side effects, symptom management, and supportive resources. Provide education regarding treatment expectations, oral chemotherapy safety, infusion therapy, and follow-up requirements. Reinforce informed decision-making and patient understanding of care plans. Provide culturally sensitive and individualized education materials and resources. Multidisciplinary Collaboration Collaborate with physicians, advanced practice providers, nursing staff, pharmacists, social workers, dietitians, financial counselors, rehabilitation services, palliative care, and community resources. Participate in multidisciplinary conferences, tumor boards, quality initiatives, and care coordination meetings. Maintain effective communication among all members of the oncology care team. Documentation & Quality Document patient interactions, care coordination activities, education, and assessments accurately within the electronic medical record. Maintain compliance with organizational, regulatory, accreditation, and oncology-specific standards. Support quality improvement initiatives, patient satisfaction efforts, and outcome tracking. Assist with data collection for oncology metrics, accreditation programs, and performance improvement activities
