Part-time, approximately 30 hours per week
Position:
Chronic Care Management (CCM) Coordinator - Registered Nurse Position Summary The Chronic Care Management (CCM) Coordinator is responsible for managing and coordinating care for Medicare and commercially insured patients with multiple chronic medical conditions. The coordinator will conduct patient outreach, develop and update care plans, document patient interactions, coordinate care between providers and specialists, track CCM minutes, and ensure compliance with Medicare CCM requirements. Responsibilities Conduct proactive outreach to eligible patients and their families. Develop, implement, and update individualized chronic care plans. Document all patient interactions and interventions accurately in the EMR. Coordinate care and communication among primary providers, specialists, and facility staff. Track and report CCM minutes and ensure all activities meet Medicare billing and compliance standards. Support smooth transitions of care and help prevent avoidable hospitalizations. Qualifications Current RN license in New York State (required). Experience in skilled nursing, long‑term care, or primary care preferred. Exceptional communication skills and phone etiquette. Strong attention to detail and commitment to accurate documentation. Ability to build meaningful relationships with patients and families in the nursing home setting. Comfort working both independently and as part of an interdisciplinary team.
Pay:
$45.00 - $47.32 per hour
