Job Description

Since 1893, Helen Keller Services (HKS) has been committed to improving the lives of individuals who are blind, visually impaired, or have combined hearing and vision loss. HKS serves in the New York City/Long Island area from its new headquarters in dynamic downtown Brooklyn and the Helen Keller National Center for Deaf-Blind Youth and Adults of Sands Point, New York, which has a network of regional offices that extends HKS' reach across the United States. Helen Keller Services for the Blind is thrilled to announce we will be expanding our Children's Learning Center to two brand-new Long Island locations in Nassau and Suffolk County! These state-of-the-art facilities will serve preschool children who are blind or have low vision, as well as multiple disabilities, autism, Down syndrome, or pervasive developmental delays. This expansion reflects HKSBs commitment to ensuring that Long Island families have access to the same high-quality early education and support services that have transformed lives at the CLC in Brooklyn. The School Nurse is responsible for assuring the health and safety of approximately 60 special education preschoolers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Plan and Conduct Childrens Learning Center Health AssessmentsSchool Nurse will identify and evaluate the school and familys health needs.

Review and assess all medical documentation of new and continuing students to determine appropriateness of placement.

Assess the families strengths and challenges during the initial intake process and assess the need for further health / medical supports from within the school and from outside medical facilities.

Where appropriate, the School Nurse will represent the CLC with other social services agencies and medical facilities.

Understand and execute a formal case review of the students medical history including general health forms, vision, neurological and other specialized reports.

Gain an understanding through formal in-person assessment of each child and through dialogue with the students parents and medical professionals to determine appropriate classroom placement and need for health care supports.

Assess the students present level of functioning and need for enhance medical interventions. Based on the students IEP evaluations and accompanying medical assessments and the students parents input, determine if such documents are accurate and adequately represent all the childs medical needs. The school nurse identifies expected outcomes for a plan that is individualized to the student or the situation.

Childrens Learning Center School Nurse Related ConceptsThe Childrens Learning Centers, School Nurse will be able to explain, demonstrate, provide instruction and reinforce all areas of each students medical needs to the Childrens Learner Center staff. This will be done through working collaboratively with the School Social Worker and the Intake and Evaluation Coordinator.

Will work on preventing and promoting health focused on the children at the school, taking into consideration the members of the school community (parents, teachers, etc.). Additionally, will provide care and assistance to our children who have fragile health. The School Nurse will help families locate resources, such as housing, medical insurance, Medicaid and other appropriate social supports. Commitment to the Students and their Families The School Nurse manage the treatments and medications prescribed by medical professionals prior written authorization and Rxs.

Control and monitor students affected by chronic and severe medical conditions, by building a network along with the families, teachers, the School Social Worker and other health professionals in the community. The Childrens Learning Center School Nurse will respect the rights of each students family to participate in decisions regarding their assessment and instructional program. The Childrens Learning Center School Nurse will effectively monitor each childs medical wellness on a daily basis.

The Childrens Learning Center School Nurse will advise the parents, the School Principal, social worker or other appropriate health professionals when students need a more specialized level of care. Advise families about their childrens health, during individual and group meetings as well as the school community on health related topics.

When communication is needed related to OCFS/communicable diseases the school nurse will disseminate that information through the ClassDoJo platform and through email.
REQUIRED
EDUCATION and/or EXPERIENCEBachelor of Science in Nursing and 2 years experience in a pediatric or school setting. Strong technical skills including MS Office, Outlook, school related applications.
