401(k) Bonus based on performance Employee discounts Wound Care Nurse Practitioner (Home Visits) Part-Time / Full-Time Opportunity Brooklyn, Queens & Occasionally Manhattan, NY About Us 180 Degrees Medical is a growing wound care and primary care practice dedicated to providing high-quality, patient-centered care throughout New York City. We specialize in the treatment of chronic wounds and provide comprehensive wound management services in patients' homes, assisted living facilities, group homes, and other community settings. We are seeking an experienced and compassionate Wound Care Nurse Practitioner to join our team and provide mobile wound care services throughout Brooklyn, Queens, and occasionally Manhattan. Position Summary The Wound Care Nurse Practitioner will independently evaluate, diagnose, and manage patients with acute and chronic wounds in home and community-based settings. The ideal candidate is comfortable working autonomously, possesses strong clinical assessment skills, and has experience managing complex wounds including pressure injuries, diabetic ulcers, venous ulcers, arterial ulcers, surgical wounds, and other chronic wounds. The provider will be expected to complete approximately 20-30 patient visits per week . Responsibilities Perform comprehensive wound assessments and develop treatment plans. Diagnose and manage acute and chronic wounds. Order and interpret laboratory studies, cultures, imaging, and vascular testing as appropriate. Prescribe medications, wound care supplies, and advanced wound therapies. Perform wound measurements, documentation, and photography when appropriate. Coordinate care with primary care providers, specialists, home care agencies, facilities, and family members. Educate patients and caregivers regarding wound prevention and treatment. Maintain timely and accurate electronic medical records. Participate in interdisciplinary case discussions as needed. Preferred Procedures (Not Required) Candidates with experience performing wound care procedures are strongly preferred, including: Sharp/excisional wound debridement Selective debridement Incision and drainage Skin substitute application Negative Pressure Wound Therapy (Wound VAC) management Compression therapy application Tissue biopsy Qualifications Active New York State Nurse Practitioner license. National Board Certification as a Nurse Practitioner. DEA registration (or ability to obtain). Minimum of 2 years of wound care experience preferred. Experience in home-based, skilled nursing, long-term care, or wound care settings strongly preferred. Excellent clinical assessment and documentation skills. Ability to work independently and efficiently in the field. Reliable transportation and willingness to travel throughout Brooklyn and Queens, with occasional Manhattan coverage. Preferred Qualifications Certified Wound Care Nurse Practitioner (CWCN, WCC, CWSP, or similar certification). Experience performing bedside wound procedures. Bilingual Spanish-speaking candidates strongly preferred. Experience using eClinicalWorks (eCW) or similar EMR systems.
