Coordine care across providers, specialists, community programs, and external healthcare facilities to ensure seamless, timely, and effective delivery of services. Provide direct professional nursing care through in person and telehealth encounters, monitor pient progress, and intervene early to prevent complicions and hospital readmissions. Educe and empower pients and families using culturally approprie methods to support disease management, trement adherence, and self-management. Analyze pient outcomes and care gaps and collabore with interdisciplinary teams to improve care coordinion processes and overall program effectiveness. Background Investigion :

Licensure For all grade levels and positions, applicants must have passed the Nional Council Licensure Examinion . In addition, they must possess a current, active, full, and unrestricted license or registrion as a professional nurse from a Ste, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United Stes. Basic Requirements for the GS-9 Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Possession of a master's degree in professional nursing; or At least one full year of professional nursing experience equivalent to the GS-7 grade level and possession of a diploma, associe degree, or bachelor's degree in Nursing from a professional nursing program. Examples of professional nursing experience managing complex case management situions by applying professional nursing judgment to assess clinical and psychosocial needs and prioritize care. Interpreting clinical da, medical records, and care plans to evalue pient stus and support decision making in care coordinion. Collaboring with interdisciplinary teams to resolve barriers to care, including access issues, resource limitions, and social determinants of health. Utilizing case management tools, da systems, and documention standards to track pient progress and ensure compliance with care requirements.
