Registered Nurse (RN) - Rockland Join the Rockaway Home Care team as a Per Diem Registered Nurse (RN) or Outpatient Nurse and make a meaningful difference in the lives of our patients in Rockland County! As a Per Diem Registered Nurse (RN) or Outpatient Nurse, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities:

Conduct in-home or community-based assessments Perform head-to-toe evaluations, including medication and diagnosis review Complete aide supervisions and provide clinical guidance Develop and update personalized Plans of Care (POC) Provide orientation to aides based on the POC Accurately document findings and maintain compliance with healthcare regulations

Requirements:

Current New York State RN license Ability to travel throughout Rockland County Medical Professional Liability insurance Comfortable using

Gmail, Microsoft Word/Excel Preferred Qualifications:

Bilingual in Spanish/English Familiarity with Medicaid, Medicare, or MLTC plans Experience in outpatient nursing, home care, or community health Strong documentation and communication skills
