Various Client Homes Position Summary:
We are seeking experienced and compassionate Registered Nurses (RNs) to join our Home Visit Concierge Service. This per diem/agency role is ideal for nurses who thrive in fast-paced, dynamic environments and can pivot quickly as patient needs and operations evolve. The RN will perform initial and reassessment assessments in the patient's home, delivering high-quality, person-centered care that ensures safety, wellness, and continuity of care.
Key Responsibilities:
Conduct comprehensive initial nursing assessments in accordance with agency policies, state/federal regulations, and payer requirements. Perform reassessments on a routine schedule or when changes in patient condition are observed or reported. Accurately document clinical findings, assessments, interventions, and follow-ups using the agency's EMR system. All documentation must be completed and submitted by the end of each visit (EOD)—timely, complete documentation is a non-negotiable requirement. Collaborate with interdisciplinary teams to ensure coordinated care plans and address patient needs in real time. Identify and report significant findings or safety concerns, and initiate appropriate follow-up actions. Educate patients and families on condition management, medications, and health maintenance strategies. Maintain professional etiquette and cultural sensitivity during home visits. Participate in onboarding, including 4 hours of documentation training and 1 day of shadowing with an experienced RN.
Required Qualifications:
Ability to work independently in patient homes and exercise sound clinical judgment.

Strong organizational and communication skills BLS Certification (Required) Comfort with mobile technology and EMR systems Five Days a Week/5 patients a day

$17 per hour base salary $100 per reassessment $130 for each Start of Care Visit.
