N/A Job Summary •The hours for this position is 3pm-11pm, M-F, no weekends Is responsible for screening all patients for medical necessity of admission, regardless of point of entry, with the use of industry approved Level of Care Criteria. Provides the care and services of patients across the continuum of their hospitalization. The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes. Qualifications A. Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write, and communicate in English. B.
Experience :
Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational, and interpersonal skills. C.
Licensure:
Current New York State Licensure as a Registered Professional Nurse, or New York State Licensure to practice Social Work in the State of New York. Certification in Case Management required 18 months after eligibility.. PRI certification preferred. Montefiore St. Luke's Cornwall is an equal employment opportunity employer.
