Job Summary:

We are seeking a dedicated and experienced RN Case Manager to join our healthcare team. In this vital role, you will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. Your expertise in clinical assessment, discharge planning, and utilization management will support optimal patient outcomes and enhance overall care quality.

Roles & Responsibilities:

Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs. Develop individualized care plans in collaboration with multidisciplinary teams, ensuring alignment with clinical guidelines and patient preferences. Coordinate inpatient and outpatient services, including discharge planning, post-acute care arrangements, and outpatient clinic follow-ups. Manage utilization review processes by evaluating medical necessity and appropriateness of services in compliance with healthcare policies. Maintain accurate medical records and documentation in accordance with HIPAA regulations and hospital standards. RN01

Required Qualifications:

Min 2 years of RN Case Manager experience required NY State license
