40 Hours Per Week Job Summary We are seeking an experienced Case Management Registered Nurse (RN) with a strong clinical background to join our orthopedic care team. This position focuses on coordinating patient care, discharge planning, and educating patients throughout their post-surgical recovery. This is a weekday-only position with no weekends, offering an excellent work-life balance. Responsibilities Coordinate care for orthopedic patients from admission through discharge Develop and implement individualized discharge plans Educate patients and families regarding post-operative care and recovery Collaborate with physicians, nursing staff, therapists, and community resources to ensure safe transitions of care Facilitate patient navigation throughout the continuum of care Document care coordination activities in Epic Utilize Careport and Parachute platforms to support discharge planning and post-acute care placement Ensure compliance with hospital policies and regulatory standards

This position does not include utilization review responsibilities, as those are managed by a separate department. QualificationsRequired Current New York State Registered Nurse (RN) License BLS Certification (AHA or ARC) Minimum 3-5 years of recent clinical experience in one or more of the following preferred: Medical-Surgical Intensive Care Unit (ICU) Emergency Department (ER) Experience with case management, discharge planning, or care coordination Experience using Epic electronic medical records Preferred Orthopedic Care Coordinator experience Familiarity with Careport and Parachute software Managed care experience ACLS Certification HC-PRI Certification and PRI screening experience Certified Case Manager (CCM) certification Schedule Monday-Friday 8-hour day shifts 8:00 AM
- 4:30 PM or 8:30 AM
- 5:00 PM No weekends 40 hours per week 13-week contract
