Full-Time At Rockaway Home Care , we understand the privilege of being invited into a family's home to care for their loved ones. We are looking for a dedicated and compassionate Full-Time Registered Nurse (RN) to join our home care team. This is a field-based role for an RN who is comfortable traveling to client homes across different counties. The RN will support clients, caregivers, and families by helping ensure quality care, proper documentation, and a positive home care experience.
Responsibilities:
Travel to client homes across assigned counties Complete nursing assessments and reassessments as needed Develop, review, and update client care plans Monitor client condition and report changes as appropriate Provide support, guidance, and supervision to caregivers Ensure care is being provided according to the client's needs and care plan Communicate with clients, families, caregivers, and office staff Maintain accurate and timely documentation Help ensure compliance with company policies and home care requirements Promote a compassionate, respectful, and professional care environment
Requirements:
Active RN license Prior home care experience preferred Willingness to travel to client homes in multiple counties Valid driver's license and reliable transportation Strong assessment, communication, and documentation skills Ability to work independently and manage time effectively Professional, compassionate, and reliable Must meet all company hiring and onboarding requirements You're not a patient, you're family.
