Job Description
Position Summary:
The Nurse Coordinator provides specialized, non-curative care to residents with terminal illness, focusing on symptom management, pain relief, comfort, and quality of life. This role also supports community relations that are integral to Shepherd Home. This position is shared with second Nurse Coordinator each working 2 ½ days during the work week M-F and includes some on call responsibilities. Position includes supervisory and scheduling responsibilities for CNAs/Aides.Education and Preferred Experience:
Active New York State RN or LPN license ( RN is preferred). A minimum of 3-5 years nursing experience (hospice, oncology, critical care (ICU), medical-surgical, or home health care experience preferred). Strong communication, decision-making, and problem-solving skills. Two or more years of supervisory experience (preferred). Empathy, compassion, and emotional intelligence skills are a must. Excellent time management and organizational skills. Proven ability to work both independently and in a team environment. Proficiency with Microsoft Office.Key Responsibilities:
Clinical assessment and symptom management Conduct pre-admission intake assessments in the community and ongoing physical assessments in the residence. Establish and monitor medication administration records, including enrolling residents in a local pharmacy and the corresponding with hospice program. Administer medications and treatments as ordered, monitor effectiveness and report changes in residents' medical status to Executive Director, and hospice program as appropriate. Perform physician-ordered treatments (e.g., dressing changes, nebulizer treatments, catheter care, drain management, and pressure injury care). Provide scheduled on-call coverage. Care planning and coordination Develop, implement, and update individualized resident care plans in collaboration with the hospice agency nurse, Hospice Medical Director, and Executive Director. Coordinate medical supplies, medications, and equipment; collaborate with pharmacists and other care providers. Coordinate resident outings with families, including transportation and documentation related to advance directives (e.g., DNR). Resident care and safety Provide bedside care as needed, including bathing/showering and other aspects of personal hygiene. Ensure safe practices for handling and disposing of bodily fluids and other biohazardous waste. Documentation and communication Maintain accurate medication administration records, physician order changes, and resident medical records. Communicate promptly with physicians regarding pain control, changes in physical status, and comfort needs. Facilitate shift-to-shift handoffs (e.g., overnight to day) as appropriate to ensure continuity of care. Resident, family, volunteer, and community support Orient residents and families, medical providers, and clergy to Shepherd Home. Provide compassionate emotional and spiritual support to residents and families. Educate caregivers and families on medication management, disease progression, nutrition, and hygiene. Communicate with and support volunteers regarding resident needs; provide volunteer training on reporting and charting resident care. Plan and facilitate educational programs (e.g., for volunteers and nursing students) After-death care and related coordination Coordinate post-death arrangements, including contacting the Medical Examiner and other required providers; attend wakes and memorials as appropriate. For organ/body donation cases, coordinate required contacts and follow established procedures following death. Human Resource Management Partner with the Executive Director to complete performance evaluations for CNAs/Aides. Support the Executive Director with interviewing and onboarding new caregivers (Aides and volunteers). Partner with the Executive Director to coordinate employee schedules specific to overnight CNA/Aide coverage.Job Type:
