MDS Coordinator (Registered Nurse)

Job Overview We are seeking a detail-oriented MDS Coordinator (Registered Nurse) to oversee accurate MDS assessments, documentation, and regulatory compliance in a long-term care setting. This role works closely with interdisciplinary teams to support quality care, appropriate reimbursement, and smooth transitions of care. Key Responsibilities Complete and coordinate MDS assessments in compliance with state and federal regulations Review clinical documentation and coding to support reimbursement and quality reporting Collaborate with interdisciplinary teams to develop and update resident care plans Utilize EMR/EHR systems for accurate documentation and audits Support discharge planning and ensure compliance with HIPAA and accreditation standards Qualifications & Skills Strong knowledge of MDS, Medicare regulations, and clinical documentation Experience with EMR/EHR systems Familiarity with ICD-10 coding, utilization review, and compliance standards Excellent attention to detail and communication skills Minimum Requirements Licensed Registered Nurse (NYS) Current BLS/CPR certification
