Per Diem Nurse Practitioner Home Health - Bible School Park, New York

Nursing degree, active Nurse Practitioner (NP) license and minimum 1+ years Home Health Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered. Deliver compassionate care in the homes of patients as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you'll engage directly with Medicare Advantage plan members across Broome County, NY—including areas such as Binghamton, Vestal, and Johnson City—to evaluate their health and ensure the best possible outcomes post-discharge or during annual check-ups.

Key Responsibilities:

Perform comprehensive annual risk assessments in patient homes Review and document prior medical history, current health conditions, and medication regimens Offer guidance and education to patients regarding their health conditions and medications Identify gaps in care, making practical recommendations for follow-up Focus on assessment—no need to create treatment plans or prescribe medications

Qualifications:

Licensed Nurse Practitioner in New York Current CPR certification (BLS required) Board Certification through AANP or ANCC Minimum of 6 months of recent experience as a Nurse Practitioner preferred MSN from an accredited nursing school Enjoy competitive compensation of $80-$90 per evaluation, plus a mileage stipend and paid administrative time.
