Job Description

Mon.-Fri. 11am-7pm (flexible schedule) $80,000-$90,000 salary $10,000 Sign on Bonus Robinson Terrace is seeking an Admissions Nurse to join our team. In this role, you will be responsible for conducting comprehensive admission assessments for new residents and hospital returns, initiating individualized care plans, and ensuring the smooth operation of the admission process in collaboration with the interdisciplinary team.
Key Responsibilities:
Conduct comprehensive admission assessments for new residents and hospital returns and initiate the development of individualized care plans. Support the Nursing Department's development and operations under the direction of the Director of Nursing (DON). Evaluate PRI assessments and coordinate the full scope of the admission process. Perform regular audits of resident records to ensure compliance with facility policies and regulatory standards. Participate in discharge planning , ensuring smooth transitions for residents. Review and update care plans when there is a change in a resident's condition. Complete required documentation , including charting, in compliance with facility policies and procedures. Prepare MDS (Minimum Data Set) assessments, care plans, and other required documentation as assigned. Uphold confidentiality of all resident information in compliance with HIPAA and company guidelines. Supervise, direct, and/or provide resident care as needed.
Requirements:
Active and unrestricted NYS Registered Nurse License. Excellent clinical assessment skills. Strong independent decision-making abilities. Leadership and supervisory skills. Proficient in MS Office (Excel, Word, Outlook). Long-Term Care Experience Preferred. Excellent communication and interpersonal skills.
