Job Description

Description The Department of Pediatrics at Stony Brook Children's Hospital is seeking Clinical Instructor/Nurse Practitioner faculty for the Division of Pediatric Nephrology. The position will be full-time, faculty, and academic at the Clinical Instructor level. Must have excellent communication, organizational, and interpersonal skills. Willingness to learn pediatric nephrology if there is no prior experience. Ability to carry out assignments independently is also required. Responsibilities & Requirements The Pediatric Nurse Practitioner will provide comprehensive care to patients of Stony Brook Children's in collaboration with an Attending Physician.
Responsibilities include:
communicate with patient(s)/families to obtain accurate information for records, address concerns and engagement in plan of care, explanation of procedures as appropriate; ensure execution of provider established plan of care including but not limited to follow up visits, specialist consultations, diagnostic lab, imaging studies and/or procedures; contribute to patient care through patient and family education, population health management, encouraging patient self-management, distributing resource literature and community service recommendations and referrals as needed; liaison with physicians and practice administration to ensure efficient and effective patient care services; educate nursing staff as appropriate regarding provider-led quality issues/metrics. The Pediatric Nurse Practitioner will participate in inpatient consultations and follow-up care in collaboration with attending physicians, assist with dialysis-related care (hemodialysis and peritoneal dialysis), depending on experience and training.
Qualifications Required Qualifications:
Master's degree in nursing or completion of a course for Nurse Practitioner; New York State license and certification as a Pediatric Nurse Practitioner. At least one year of clinical experience working in Pediatrics is required.
Preferred Qualifications:
Five or more years of clinical experience working in Pediatrics. Experience with outpatient care for pediatric or nephrology patients, including chronic kidney disease, hypertension, glomerular disorders, electrolyte disorders and post kidney transplant follow-up.
