Job description: Come join a friendly and compassionate home care agency that is built upon providing our clients with the best life possible. We offer paid orientation and competitive pay . The ideal candidates will have homecare experience and be responsible for: Conducting admission assessments and biannual reassessments for patients. Establishing and providing a plan of care for the patient based on results of the nursing assessment visit, client needs, and physician orders. Providing education to patient/family and paraprofessional staff. Conducting as-needed visits to patients who require additional support. Communicating changes in patient's condition, treatment, and meds directly to the agency.
