Responsibilities:
Observe and document resident changes in condition accurately per policy Ensure continuity of care by providing direct care and supervising licensed practical nurses, nursing assistants, and nursing students Monitor, treat, and report resident pain utilizing pain management techniques Administer medications following medication rights to reduce medication errors Give thorough reports to oncoming shifts by documenting in resident records and 24-hour reports Assist with medical procedures as needed Maintain role as resident advocate with a focus on the mission of the facility Implement plan of care based on assessments and goals established by the interdisciplinary care team Ensure residents at end of life or on hospice services are kept clean, dry, and comfortable Educate patients and family members on treatment and care plans, and answer their questions
