RN Quality Educator 3.3 3.3 out of 5 stars Webster, NY 14580 $46,924.80 - $90,937.00 a year - Full-time UR Medicine Home Care 37 reviews $46,924.80 - $90,937.00 a year - Full-time The RN Quality Educator will assist in onboarding new team members, role modeling of best practices of education workflows, iCare values and the implementation of process improvements within the quality and education department. Assist in the coordination of the education of all new clinical staff in collaboration with quality education team and field clinicians at established points in time to assure accurate training, education, and understanding of agency policies, procedures, and standards. The position serves to provide appropriate support and promote quality patient care, positive patient outcomes, and staff retention.
Schedule:
Monday - Friday, Days 8am to 430pm Qualifications Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
Education:
Nursing Degree from an accredited school of nursing.
Experience:
Two (2) or more years of experience working in Home Health, working with Medicare Regulations and/or experience in home care criteria and processes related to Managed Care. OASIS certification within one year of hire. Licensure, Certification, and/or Credential Requirements Currently licensed as a Registered Nurse in the state of New York. Valid NY State Driver's License and reliable transportation. Current BLS CPR certification or ability to obtain upon hire. Responsibilities Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations: Work with education team in the creation and coordination of clinical education delivery. Works collaboratively with Clinical Operations, Quality, Education, Clinical Specialists and Corporate Compliance, to ensure that a comprehensive, standardized education program is provided to all clinical employees. Integrates concepts of Adult Learning Principles. Seeks training resources and tools to stimulate creative thinking, create meaningful and effective educational experiences, and build expertise and confidence through progressive learning activities. Work with education team to track NEO progress throughout clinical orientation and support individual clinicians. Collaborates with clinical staff, clinical supervisors, and managers to facilitate competence in skill development and critical thinking. Notify managers of discrepancies and inconsistencies. Serve as a resource to staff related to clinical assessment of patients and the need for appropriate interventions. Collaborate with clinical supervisors/managers. Communicate staff performance issues to clinical manager. Participate in department and team meetings to provide education to staff on best practice, documentation, and regulatory updates. Perform joint home visits with staff requiring additional education and identified high risk cases to assure best clinical practice, OASIS and POC documentation competency. Proficient use of Home Care electronic medical record and Decision Support software. Prepare, distribute and present reports related to utilization of care and clinical outcomes as needed. Develop and maintain education related to the following, including, but not limited to: mandatories, competencies, skills labs, and focused co-visits. Performs comprehensive chart audits as needed to identify performance improvement needs. Develop appropriate education and monitoring to ensure clinical excellence and quality patient care objectives are achieved. Monitors key agency reports to identify actual/ potential performance improvement needs. Other duties as assigned.

Outlook, Word, Power Point and Excel E-Record (electronic medical record systems) Office equipment, such as computers, printers, copiers, and fax machines, as well as multi-line phone systems Intranet/Internet Work Environment Exposure to hazardous conditions/ materials is negligible. Travel throughout service area year around. Environmental settings will vary between patient homes, remote environment, and office.
Salary Range:
$46,924.80 - $90,937 #URM2
