FIELD NURSES NEEDED
for Home Care Agency Job description: Able Health Care Service is a leading home care provider that offers Home Health Aide/Personal Care Aide services to patients in need. Our home care workers support and assist patients with carrying out activities of daily living. Able Health Care has been in business for 48 years and highly values and appreciates our clinical team.
Responsibilities:
Looking for the ideal candidate that has experience working in CHHA environment. HCHB experience is a plus. Knowledge of OASIS, clinical outcome measures, federal conditions of participation and NYS DOH regulations related to Home Health Care Provides medical oversight for clients in their homes
  • including but not limited to assessments, obtaining MD orders, monthly contacts and maintaining an appropriate Plan of Care.
Responsible for coordination, and supervision of the agency's paraprofessional staff. Conducts reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff Qualifications NYS licensed and currently registered as a Registered Professional Nurse. Candidates with previous Home Care experience required Thorough knowledge of the scope of practice of professional and paraprofessional healthcare. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Excellent organizational writing, communication, and interpersonal skills. $50-150 depending on type of visit
Full-time:
$90,000
  • 110,000; prorated for part-time Work setting In-person; Field EOE
