"Navitas Healthcare, LLC" is seeking an experienced RN

• Adult Care / Home Care Nurse to provide high-quality nursing services to patients in their homes throughout the Bronx Borough.

The RN will deliver comprehensive patient-centered care, conduct clinical assessments, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes.

Primary Responsibilities:

Assess, plan, implement, and evaluate individualized patient care plans Provide direct nursing care to adult home care patients Administer prescribed medications and treatments Perform wound care, IV therapy, catheter insertion, and post-operative care Monitor patient vitals and report adverse reactions or changes in condition Educate patients and families regarding treatment plans and home care instructions Maintain accurate documentation, patient files, and health records Collaborate with healthcare providers on discharge planning and continuity of care Ensure adherence to nursing standards, infection control, and clinical protocols Utilize OASIS documentation and

EPIC EMR

systems as required

Experience & Requirements:

Minimum 2 years of recent Med/Surg experience required Home care nursing experience required OASIS knowledge required

EPIC EMR

experience preferred Must be comfortable traveling to patient homes within

Bronx Borough Education Requirements:

Associate Degree in Nursing (ADN/ASN) from an accredited institution required Licensure /

Certification:

Active New York State RN License required Primary Source Verification of RN License required AHA BLS Certification required
