YOUR ROLE AS A REGISTERED NURSE CASE MANAGER

(RN) As a Registered Nurse Case Manager (RN) at VNA of Ohio Home Health, your day-to-day expectations will include conducting comprehensive patient assessments and developing individualized care plans that address the specific needs of each patient. You will be responsible for coordinating services with an interdisciplinary team, ensuring that all aspects of care are efficiently managed. Daily tasks will involve monitoring patient progress, conducting home visits, and providing education to patients and their families about health management and resources available to them. Additionally, you will maintain accurate documentation and communication with healthcare providers to facilitate continuity of care. Participation in team meetings and ongoing professional development will also be essential to staying abreast of best practices and innovations in home health nursing.
