YOUR ROLE AS A REGISTERED NURSE CASE MANAGER
(RN) As a Registered Nurse Case Manager (RN) at VNA of Ohio Home Health, your day-to-day expectations will include conducting comprehensive patient assessments and developing individualized care plans that address the specific needs of each patient. You will be responsible for coordinating services with an interdisciplinary team, ensuring that all aspects of care are efficiently managed. Daily tasks will involve monitoring patient progress, conducting home visits, and providing education to patients and their families about health management and resources available to them. Additionally, you will maintain accurate documentation and communication with healthcare providers to facilitate continuity of care. Participation in team meetings and ongoing professional development will also be essential to staying abreast of best practices and innovations in home health nursing. Your role is vital in enhancing the patient experience and outcomes within our community.

WHAT YOU NEED TO BE SUCCESSFUL
To thrive as a Registered Nurse Case Manager (RN) at VNA of Ohio Home Health Operations, LLC, a diverse skill set is essential. Candidates should possess an associate or bachelor's degree in nursing, with a BSN preferred, and hold an active RN licensure in Ohio. At least two years of clinical nursing experience is required, including a minimum of one year in a home health setting. Successful candidates will demonstrate expertise in patient assessments, care plan development, and evidence-based nursing interventions. Strong communication skills are crucial for effective interaction with patients, families, and interdisciplinary team members, promoting clear and compassionate care.
