Job Description
Premier Nursing, LLC is actively hiring for multiple PRN RN Case Managers to join our professional team of clinicians in the Butler/Clermont/Montgomery or Warren county areas. Service areas needing coverage includes Hamilton, Fairfield, Lebanon, Loveland, Mason, Middletown, Milford, Monroe, and West Chester. The RN will require 1 year of home health experience in Case Management and Oasis. RN will develop a treatment plan based on the adult comprehensive assessment with Physician for improving patient care outcomes, in addition, working with a multidisciplinary team for direct patient centered care. Premier Nursing is CHAP accredited and recognized in Home Care Compare for patient Quality in care and patient satisfaction. We are looking for the right RN candidate who has knowledge and experience with education in disease management, surgical aftercare, and other post acute care needs to facilitate patient's transitioning back into their homes. The applicant must have strong computer skills, strong documentation skills supporting medical necessity in care, have critical thinking, and an understanding of Medicare guidelines and eligibility.
