Job Description

A Brief Overview RN Patient Navigators provide high-touch customer service to UH customers, building strong relationships while helping them navigate the healthcare system. What You Will Do Coordination of care and access of specialty and general medicine care and screening. Community resource referral, patient education, and navigation/entry point in to UH. Actively participates in the process for operational improvement and quality improvement. Involved in learning and self-improvement. Provides leadership through communication, education and management. Provides advocacy for the patient and family. Tracks interventions and outcomes of patients and families who access UH. Initiates and maintains positive relationships with co-workers and physicians Additional Responsibilities Demonstrates commitment to the mission, vision and values of UH. Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education (BSN) Bachelor's Degree in Nursing (Required) Work Experience 5+ years in clinical medicine (Required) Knowledge, Skills, & Abilities Fluent in English both verbally and written (Required proficiency) Comprehensive understanding of System and community resources and requirements for access. (Required proficiency) High integrity level and ability to maintain confidentiality of sensitive data (Required proficiency) Experience working multiple healthcare data sources that include Electronic Medical Record application(s), registration and scheduling systems, and medical claims systems. (Required proficiency) Experience facilitating telephonic patient outreach. (Required proficiency) Understanding of healthcare industry trends and Federal and Commercial insurance (Required proficiency) Understanding of System and comments resources and requirements for patient access (Required proficiency) Comprehensive understanding of various disease processes, and requirements and challenges of self-care. (Required proficiency) Excellent verbal and written communication skills. (Required proficiency) Proven time management and organizational skills (Required proficiency) Demonstrated excellence in customer service with all stakeholders (patient, employer staff, providers, etc…). (Required proficiency) Demonstrated ability to apply concepts, utilize sound judgment and work independently. (Required proficiency) Ability to work within a team environment (Required proficiency) Ability to handle multiple initiatives and priorities within a fast-paced environment. (Required proficiency) Good computer skills with the ability to utilize Microsoft Office Products.
