In this vital role, you will coordinate comprehensive patient care plans, facilitate smooth transitions across home healthcare settings, and ensure optimal health outcomes. Your expertise in acute and post-acute care, along with your strong clinical assessment skills, will drive patient-centered solutions. Responsibilities Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs. Develop and implement individualized care plans that align with clinical protocols and patient preferences. Coordinate inpatient and outpatient services, including discharge planning and post-acute care arrangements. Manage utilization review and case management processes to optimize resource use while maintaining high-quality patient care. Collaborate with multidisciplinary teams Maintain accurate medical records and documentation in compliance with HIPAA regulations and medical terminology standards. Facilitate diagnostic evaluations, medical record reviews, to support effective patient management. Engage in patient service activities such as intake assessments, patient education, and coordination of outpatient clinic visits. Support home health experience initiatives by ensuring seamless communication among providers, patients, and families during transitions of care. Participate in quality improvement efforts Demonstrated background in home health care. Strong knowledge of infectious disease care, geriatrics, behavioral health, pediatrics (PICU), and post-acute outpatient services. Familiarity with managed care processes including utilization management and case management practices. Extensive experience conducting patient assessments Excellent understanding of medical terminology, HIPAA compliance standards, and discharge planning procedures.
