Full-Time - 4 10-hour shifts including every other weekend Job Summary Capital City Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our Hospice Admissions team. This role is responsible for conducting pre-admission assessments, educating patients and families, and initiating hospice care services. The ideal candidate is patient-focused, clinically strong, and committed to delivering high-quality end-of-life care. Key Responsibilities Perform pre-admission evaluations to determine eligibility and appropriateness for hospice services Educate patients and families on hospice philosophy, care options, and services Initiate admission process and obtain informed consent Conduct comprehensive nursing assessments Develop and implement individualized patient-centered care plans Provide interventions that support comfort, dignity, and quality of life Collaborate with interdisciplinary care team Ensure compliance with quality standards and regulatory requirements Required Qualifications Graduate of an accredited Registered Nursing (RN) program Active Registered Nurse (RN) license in the state of practice Current CPR certification Ability to make independent clinical decisions Strong communication and interpersonal skills Ability to work effectively with patients, families, and healthcare professionals Knowledge of quality assurance and performance improvement processes Preferred Qualifications Minimum 1 year of hospice or home health experience Experience with patient admissions and care planning Compensation & Benefits Competitive salary (based on experience) Medical, dental, and vision insurance (effective first of the month) 401(k) with company match Paid time off (PTO) and paid holidays Mileage reimbursement Tuition reimbursement Hospice certification support with salary increase Employee referral bonus program Merit-based increases Employee discount programs Work Environment & Culture No on-call requirements Low patient caseloads Supportive and collaborative team environment Focus on work-life balance and employee well-being Mission-driven organization rooted in Ohio communities About Capital City Hospice Capital City Hospice is proudly Ohio-based and dedicated to serving Central Ohio communities.
