Job Description

Trotwood Health & Rehab is currently seeking an
MDS COORDINATOR
to oversee the RAI process and completion of the process for residents in accordance with all laws, regulations, and Company standards. SKILLS and
ABILITIES
Experience:
Minimum of 2 years of MDS experience CERTIFICATES and
LICENSES RN
degree from accredited college and in good standing with current licensure board. RAC-CT preferred but not required Must have CPR certification upon hire or obtained during orientation. CPR certification must remain current during employment.

OUR WORKPLACE
We are a long-term care skilled nursing facility operating 24/7. Our facility is committed to providing the highest quality of care and services assuring human dignity and quality of life for our residents, their families, and our employees.
ESSENTIAL FUNCTIONS
Must be knowledgeable of nursing practices and procedures as well as the laws, regulations, and guidelines governing nursing functions in the long-term care facility. Must be able to assist in the care needs of the residents as needed. Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel. Must have the ability to implement the programs, goals, objectives, policies, and procedures of the Nursing department. Must be able to keep current on best practice and current standards in the related clinical field. Must be able to investigate and direct resident inquiries or complaints to appropriate medical staff members. Must be able to coordinate communication between residents, family members, medical staff, administrative staff, or regulatory agencies. Must understand and follow company policies. Must be able to read, write, speak, and understand the English language. Must be able to understand the financial concepts and implications of RAI process, including RUGs and case mix components. Must be able to interview residents or their representatives to identify problems relating to care. Must be able to explain policies, procedures, or services to residents using methods in which the resident or representative understands the content.
