Job Description

The Registered Nurse shall make the initial evaluation visit. Assesses the client's condition and needs through collection of health data, the recognition, interpretation and reporting of symptoms and psychological responses, as well as the identification of resources available in the home setting. Initiates the plan of treatment and necessary revisions.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Formulates, modifies and carries out, and regularly reevaluates client care plans which prescribe nursing actions toward achievable goals, according to client needs and coordinates services.
  • Implements, with consideration of the dignity and desire of the client, the plan of care, organizing and participating in direct client care, performing treatments, administering medications, initiating intravenous therapies, and obtaining venous blood for laboratory blood studies.
  • Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills
  • Maintain safe, therapeutic environment for the client.
  • Implements the medical regimen by carrying out physician's orders, clarifying these orders as necessary, and informing the physician of changes in the client's condition.
  • Conducts patient care conferences on patients assigned to his/her care.
  • Takes on-call duty nights, weekends and holidays, as assigned.
  • Sets up goals, teaching plans, implementing a practical realistic teaching program in the home.
  • Assesses the client's/families ability to safely self-administer and monitor home therapies.
  • Maintains detailed records of nursing actions, client's progress, responses to therapy and teaching, so that implementation of plan of care can be documented and evaluated.
  • Delegates client care responsibilities to other nursing personnel based on client need and staff capabilities.
  • Supervises and evaluates care given by Company nursing and ancillary personnel.
  • Provides education to other Company nursing and ancillary personnel.
  • Counsels and instructs Company personnel as needed in techniques and equipment utilized in home therapies.
  • Initiates cardiopulmonary resuscitation (CPR) and institute other emergency procedures when necessary.
  • Communicates information effectively to appropriate personnel to facilitate continuity of care.
  • Conforms to client facility, nursing service department and home care product provider protocols and policies.
  • Accepts accountability for own practice.
  • Maintains competency by participating in continuity education programs.
  • Complies with all Company policies and procedures, including but not limited to the Company Compliance Program.
  • Adheres to ANA Code of Ethics.
  • Recognize your role as part of the Quality Assessment and Performance Improvement (QAPI) efforts of the agency
  • Attend trainings to build understanding and capacity to undertake QAPI work
  • Carry out QAPI roles and responsibilities as assigned
  • Follow established policies and procedures in support of QAPI efforts
  • Look for and share with leadership ideas for improvement in the organization
  • Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success
  • Support QAPI efforts both verbally and non-verbally (i.e. via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
  • Supportive of individual and Company performance improvement initiatives.
  • Maintain confidentiality with patient, employee and Company sensitive information.
  • Comply with accepted professional standards and practices.
  • Participates in the process of orientation and training of new nurses as needed.
  • Perform other duties as assigned by the Company or the Supervisor.
