Job Description

COMMUNITY RESIDENCE NURSE
Anne Grady Services is seeking a dedicated and compassionate Community Residence Nurse to join our Community Programs team. As a Community Residence Nurse, you will play a vital role in providing nursing care and support to individuals with intellectual disabilities within our community residences. Your responsibilities will include implementing physician's orders, providing general nursing care, and ensuring that our residents receive the highest standard of care in a safe and supportive environment.
KEY RESPONSIBILITIES
Medical Care:
Carry out physician's orders and Interdisciplinary Team directives, ensuring timely and accurate administration of medications and other nursing tasks in compliance with Licensure, Medicaid, and other applicable regulations.
Documentation:
Complete all necessary documentation in accordance with regulations and organizational policies.
Collaboration:
Participate in Interdisciplinary Team meetings, MUI (Major Unusual Incidents) investigations, and health services consultations. Communicate effectively with physicians, pharmacy staff, and other healthcare providers as needed.
Resident Support:
Promote active treatment, dignity, independence, and autonomy for individuals served. Conduct on-site visits to assess residents' health status and ability to self-administer medications.
Training & Supervision:
Ensure timely completion of re-certifications and provide on-call delegation for all ICFMR (Intermediate Care Facilities for Individuals with Intellectual Disabilities) homes.
Teamwork:
Work cooperatively with Community Programs Coordinators and Managers to ensure integrated service delivery. Exchange information with other staff to ensure continuity of care.
Qualifications:
Current Ohio license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Minimum of six (6) months of experience working with individuals with intellectual disabilities. At least one (1) year of nursing experience. Current CPR/First Aid certification. Ability to work independently, meet deadlines, and prioritize work effectively. Strong ability to direct support staff and function as part of a management team. Willingness to complete Board-approved Delegation training and learn relevant regulations and guidelines. Physical ability to perform tasks such as pushing, pulling, bending, sitting, standing, and stooping for extended periods, as well as lifting, transferring, and repositioning individuals regularly and spontaneously.
