Home Care Assessment RN - Full Time Allen County, OH| Gas Card + Maintenance Stipend| M-F Flexible Scheduling| Non-Clinical | New Grads Welcomed! Looking to step away from bedside nursing while still using your RN skillset in a meaningful way? This Home Care Assessment RN opportunity offers flexibility, autonomy, and a strong work-life balance in a non-clinical setting. In this role, you'll complete patient assessments to evaluate eligibility and continued need for Medicaid-funded in-home support services. You'll also create and update care plans for Home Health Aides and Personal Care Assistants to help patients safely remain independent at home. This is NOT a skilled nursing/home health role — no wound care, med passes, hands-on treatment, or bedside responsibilities involved.

Responsibilities:
  • Conduct initial and follow-up patient assessments
  • Evaluate eligibility and level of care needs
  • Develop and update patient care plans
  • Maintain documentation and compliance requirements
  • Coordinate with patients, caregivers, and internal staff
Qualifications:
  • Active OH RN license
  • Valid driver's license
